Qualification

Experience
- 1–2 Years of experience in a hospital, healthcare, or social service setting preferred
Job Responsibilities
- Provide counseling and emotional support to patients and their families
- Assess patients’ social, financial, and psychological needs
- Coordinate with doctors, nurses, and other healthcare professionals for patient care planning
- Assist patients in accessing healthcare services, government schemes, and community resources
- Conduct patient and family education programs
- Maintain accurate case records and documentation
- Support discharge planning and follow-up care arrangements
- Participate in community outreach and awareness programs
